Short answer: A cavity is the end result of an acid attack on your enamel that your body couldn’t repair fast enough. Bacteria in plaque feed on sugars and starches and produce acid as a byproduct; that acid pulls minerals out of your enamel faster than your saliva can put them back. Fluoride doesn’t kill the bacteria — it makes the mineral-rebuilding side of that tug-of-war stronger, so enamel can repair itself before a cavity forms.

The Demineralization/Remineralization Cycle
Your enamel is losing and regaining minerals constantly throughout the day — this is normal and it’s happening right now. Every time you eat or drink something with sugar or refined carbohydrates, the bacteria in plaque metabolize it and release acid, which softens and pulls minerals (mainly calcium and phosphate) out of the enamel surface. That’s demineralization. Between meals, saliva neutralizes the acid and redeposits minerals back into the enamel — that’s remineralization.
A cavity forms when the scale tips too far toward demineralization for too long: frequent snacking or sipping sugary drinks throughout the day doesn’t give saliva enough recovery time between acid attacks, and the enamel surface eventually breaks down into an actual hole rather than just a weak spot.
Why Frequency Matters More Than Quantity
This is the part that surprises people: it’s not really about how much sugar you eat, it’s about how often your teeth are exposed to it. Eating a dessert in one sitting causes one acid attack. Sipping a sugary coffee or soda over three hours causes a continuous one. That’s why grazing on snacks or slowly nursing sugary drinks tends to be harder on teeth than eating the same amount in one sitting.
What Fluoride Actually Does
Fluoride works in a few specific ways. It gets incorporated into the enamel structure during remineralization, forming fluorapatite, which is more acid-resistant than the mineral it replaces. It also attracts calcium and phosphate ions to weakened spots on the enamel surface, speeding up repair. At higher concentrations (like in a prescription-strength toothpaste or an in-office varnish), it can also interfere with how cavity-causing bacteria produce acid in the first place.
This is also why the “swish and spit, don’t rinse with water afterward” advice on toothpaste labels matters — rinsing right after brushing washes away the concentrated fluoride before it has time to work.
Where Fluoride Comes From
Most people get fluoride from a combination of toothpaste, fluoridated tap water (if your municipality fluoridates), and in-office treatments like varnish. Bottled water and well water often don’t contain meaningful fluoride, which is worth knowing if that’s your main water source — it’s a reasonable thing to bring up with your dentist or your child’s pediatric dentist.

Can a cavity heal on its own without a filling?
Very early demineralization — a white spot with no cavitation yet — can sometimes be remineralized and stopped before it becomes an actual cavity, especially with fluoride treatment. Once the enamel surface has physically broken down into a cavitated hole, it can’t repair itself and needs a filling.
Is fluoride safe?
At the levels used in toothpaste and community water fluoridation, major health and dental organizations consider fluoride safe and effective for cavity prevention. The main thing to manage in young children is swallowing too much toothpaste, which is why a pea-sized amount and supervision are standard advice for kids under six.
Do I need fluoride if I don’t eat much sugar?
It still helps — the acid-buffering and remineralizing effect works regardless of what caused the demineralization, and factors like dry mouth or acidic foods and drinks (coffee, wine, citrus) can weaken enamel independent of sugar intake.
